Submit Guardianship Notice

AUTHORITY: ORS 125.060 AND ORS 125.082(3)(D), AS AMENDED 2021

Guardians in Oregon are required to submit notice to Disability Rights Oregon if any of the following conditions are true:

  1. The respondent or protected person is a resident of a mental health treatment facility.

  2. The respondent or protected person is a resident of a residential facility for individuals with developmental disabilities.

  3. The guardian intends to place the respondent or protected person in a mental health treatment facility.

  4. The guardian intends to place the respondent or protected person in a residential facility for individuals with developmental disabilities.

Guardians Represented by an Attorney

Beginning on January 1, 2022, all guardians represented by counsel in these types of cases must submit the required notice identified in ORS 125.082(3)(D) electronically through the following form. Disability Rights Oregon encourages and appreciates all filings being submitted electronically.

Guardians Not Represented by an Attorney

If the guardian is not represented by counsel, you may also use this form to submit notice or you may submit notice by mailing the document to:
